What telehealth means
Telehealth is the delivery of healthcare services using interactive audio, video, or data communications. You will interact with your clinician through secure messaging, and where required by state law or clinical judgment, through live video.
Benefits
Increased access to care, reduced travel, faster review and prescription, and continuity of care through ongoing messaging.
Risks & limitations
Telehealth has limitations: a clinician cannot perform physical examination, draw labs, or directly observe non-verbal cues to the same extent as an in-person visit. Technology can fail. In rare cases, this may affect diagnosis and treatment.
If your condition cannot be safely assessed by telehealth, your clinician will refer you to in-person care.
Emergencies
WeightLoss Plus is not an emergency service. For medical emergencies, call 911 or go to your nearest emergency room.
Your rights
You have the right to withdraw consent at any time and to receive a copy of your medical record. You have the right to request care from a different provider where available.
