HIPAA Notice of Privacy Practices

This notice describes how protected health information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Uses & disclosures of your information

We may use and disclose your protected health information ("PHI") for treatment, payment, and healthcare operations, including:

  • Treatment. Sharing PHI with licensed clinicians and pharmacies involved in your care.
  • Payment. Processing payment for the services you receive.
  • Operations. Quality improvement, training, and business administration of the practice.

Your rights

You have the right to:

  • Inspect and copy your medical record
  • Request amendments to your record
  • Receive an accounting of disclosures
  • Request restrictions on certain uses and disclosures
  • Request confidential communications
  • Receive a paper copy of this notice
  • File a complaint with us or with the Department of Health and Human Services

No marketing without authorization

We will not use or disclose your PHI for marketing purposes without your written authorization. We will not sell your PHI. Period.

Breach notification

If a breach of your unsecured PHI occurs, we will notify you in accordance with applicable federal and state law.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at privacy@weightlossplus.com or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

Effective date

This notice is effective May 1, 2026, and remains in effect until replaced.
